After my fathers heart surgery he refused to stay in the inpatient rehabilitation center but he was unable to take care of himself. In addition, he was suffering from "vascular dementia" which was not in our plan either. I was totally unprepared for this. Care to Continue was the one solution that didnt threaten his independence and allowed my family to have a normal life during his slow recovery. Tyler made a custom plan that perfectly suited our needs. The staff was always courteous and respectful and we were pleased that they were thoroughly screened before being sent into his home. I highly recommend this service and I am thankful it will be there if we need them again.
