I have been working as a full-time Physical Therapist of Trilogy/Paragon Rehab at this PLHC location for the last 4 years. I have had a distinct honor and privilege to work with some of the most outstanding nurses / director of nursing / executive directors and of course our Therapy staff. Sure as time passes by, people move on for various reasons and people have -ve things to talk about ( which is evident from various reviews on other websites ). Every single day, we have to deal with different people ( personalities ), therefore we might be making mistakes, as its hard to please everyone! However, what I have experienced and being a staff member, that we as a company strive hard to #givingback to the community, #ServiceFirst motto, #attitudeofgratitude #applybestjudgement. We are not perfect in any sense, however to judge the entire facility / company based on 1-2 employees is not a fair judgement anyway. The facility has all it takes to be 5-Star in this Noblesville area. I might be biased, but not the residents and their families who keep coming over and over again.... not just re-admissions, but for out-patient therapy as well. We have countless number of people who came here just because someone else told them. That makes we wonder, "obviously we are doing something right"! We have the best trained therapy staff on board - -- please come see us some day and you will figure out! :)

I have been so pleased with Prairie Lakes and the care my mother is receiving there. Jess was the first person I met with and she was so extremely helpful with getting my mom all setup there. Her caring attitude makes everyone feel so welcome there! The entire staff is wonderful and so caring! The facility is gorgeous and very homey! My mothers room is spacious and beautiful. The food is excellent and they even have made to order breakfast. My favorite part of Prairie Lakes is the many activities that the residents can attend. I love how busy my mom is, going to pet parades, bingo, choir, and so much more. They do a great job keeping the residents busy and stimulated! I am so pleased that my mom is at Prairie Lakes where I know she is being well cared for and I love that I am only five minutes away so I can spend a lot of time with her.

My observations from Prairie Lakes from back in March was the best. You can tell Staff care about their individuals under there care. They are emotionally connected with thier individuals. The night before i was there, they had one of thier individuals under their care. They felt the loss just as they lost one of their own family members. This is a prime example of the care they give. Grand mothers care while here is been great. Enjoys pictures on her tv. Everything has been great.

Looks nice on the surface, but this place is not equipped to take care of my dog. My step-dad was there for a week and they wanted him out because they kept forgetting about what he needed and said they didnt have the staff to take care of him. So WHY are they a nursing home facility?????

This is such a beautiful health campus that has so much to offer its residents! The food is great and the campus itself is very clean and neat. The staff also offers exceptional care to its residents! This campus deserves an A+!
