We filed a complaint with the Arizona Medical Board, but the doctor was not held accountable. There was no feedback from them or any transparency. We could not respond. We used an anonymous egg donor who had 2 healthy children of her own and donated 3 times before us to other couples resulting in healthy pregnancies. We used a gestational carrier who had 2 healthy kids of her own. The egg donor was provided by our IVF doctor and the gestational carrier was found through an agency in phoenix that the doctor works with. He was responsible for all medical care to both the donor and gestational carrier. He was responsible for their screening. Our daughter was born with Spina Bifida Myelomeningocele. Our IVF doctor did not PREP, PRESCRIBE or VERIFY (see below) which he confirmed to me in a phone conversation. He said she should get folic acid from her diet. He ended the phone conversation stating there was another child with SB and the child turned out fine (for me not to worry). No accountability, no change. The Gestational Carrier stated in front of other OBs after leaving his practice (she was released from his care at 10 weeks) that he never prescribed. In addition, I have a text from her stating this. She lost 10% of her body weight while under his care (she said due to fertility meds which I found out later at the midwife appt after she left his care). Multiple persons in the medical field have told us that this should not have happened that he was negligent (but wont stand with us). In our daughter’s case, Spina Bifida was completely avoidable with the right amount of folic acid before and during a planned pregnancy. Our daughter’s condition was the result of lack of standard of care. He is in this business and he is the professional. He should have: 1. Prepped and Prescribed Prenatals for the Egg Donor and Gestational Carrier. Prescribe approved prenatals including additional Folic Acid at least 2 months before any procedures. The IVF doctor who enlists these donors should be responsible for their care starting at least 2 months before any procedure. (reference: mayo clinic spina bifida prevention) 2. Verify. Measure Folic acid absorption levels or folic acid deficiency via blood work. (1) Check the homocysteine levels. If a person carries the MTHFR mutation and the mutation is active, homocysteine levels would be elevated. (2) Measure the vitamin deficiencies such as D, B12, folate, etc. If any issue is found, the procedure should be stopped. He never did any PREP, PRESCRIBE or VERIFY. In todays day and age, with the current body of knowledge on folic acid and testing available, this should never have happened. His sole concern was getting the SART/CDC statistic towards counting as a pregnancy which he achieved. This was a planned pregnancy and he is the professional. That the AZ Medical Board did not hold him accountable is disgusting. Our child has to live with this condition for the rest of her life.

Do not even bother filing a complaint. There are multiple departments where your complaint could be shot down. Do it if you are tough and have the time, but be ready for a disappointment. Hope for the best, expect the worst. I filed a complaint against MISTER Lester A Epstein. I wish I could give zero stars. This was the absolute worst experience I have ever had with a physician. MISTER Lester A Epstein (I will not dignify this clown act with a title) was apathetic, moody, shifty, did not make direct eye contact, and dragged his feet while I was very, very sick. He diagnosed me with Graves Disease. I had three malignant, cancerous tumors. I was pale, sweaty (diaphoretic), and in great pain. He told me to get the "chip off my shoulder." He was fully aware of the cancer as evidenced by his constant correspondence and documentation with the imaging center. he wrote many notes stating I was healthy enough to work. I worked 2 jobs, was in pain, and did not know exactly how sick I was. I ended up losing my job due to absenteeism. All MISTER Epstein could say is, "They cant do that." Wake up, Bozo, it is a right to work state, and they did a pretty good job of doing it. MISTER Epstein FINALLY ordered the surgery. Then, he forgot about me. Plain and simple, he FORGOT all about me. He had no idea of the date of my surgery, he never requested a pathology report, he never documented on me during the 6 weeks after my surgery, he never asked about the cancer, he had no idea what cell of cancer it was, he never prescribed Synthroid from September to mid November, he never even picked up the telephone to see how I was doing. I called him!!!! He said, "You mean the surgeon (MISTER Devon Gujrathi) sent you home with no Thyroid and no replacement?!!!!! You mean you already had the surgery??!!!!" He was completely in the dark. Epstein was the referring physician. Why did he know absolutely nothing? The surgeon might as well have taken feces and smeared it in Epsteins face. When MISTER Epstein finally got around to seeing me, he said to my mother, "Gee, I suspected Cancer all along." Thanks a lot MISTER Epstein! Thanks a lot! I was in great pain. However, I am sure you billed that insurance company. Do you collect capitated fees? Please do not see this clown. I filed a complaint with the Arizona Medical Board against him for neglect and malpractice, in August of 2014. See ANYONE else. heck, I would even suggest driving down to 20th and Broadway and seeing some Big Momma before I would subject you to this neglect. Seriously, see a real endocrinologist. Basically, the board approved this behavior. When I said that I could not believe that the board could come up with such a decision as dismissal, an employee said, "Well, they look at every statute." These charlatans are covered and protected, and they know it!

Dr Daniel Lieberman performed a fusion with a cage in March of 2012. In May, 2013 I began experiencing low back pain. Dr Gaza, one of his associates, had informed me that I had another herniated disc and I attributed the pain to that. The pain became increasingly worse and we came back from MN. I had an MRI and saw Dr Lieberman on September 8, 2013. He informed me that he no longer accepted my insurance. He showed me on the MRI that the rod he had inserted broke. (I found out later from another MD that the rod broke because the fusion did not fuse.) I explained my extreme pain and said "Isnt there anything you can do to help me?" By this time I was crying. He said "You cant afford me.". gave me a prescription and said he would refer me to another MD., an MD that I couldnt get an appointment with until December 5, 2013. I found his behavior to be very rude and uncaring.

DR. Sanvanson issued a group of referrals for me that were totally unnecessary. Even the folks that work there are amazed. He told me my physical should blood in my stool......wrong...he issued a colonoscopy referral.....the results of the physical I had just had showed no such thing. He called for a PSA........I cannot have a prostate problem...any doctor will tell you that....Im a Klinefelter baby. He said I had a severe kidney problem...wrong...Dr Rai will tell you that is totally wrong. He said I had excessive protein in my urine....etc. etc. etc. This guy is dangerous !! I talked to another of his patients...........they said they went once.....never again !!! Ive told everyone I see................this guy is "quackable".
