Heartland Health Care Center of Champaign is far and away the best transitional care facility in the area. This observation after having the unfortunate opportunity to compare others. Heartland is patient and family centered in its approach to care. Communications are paramount, with staff and administration informing us immediately of significant changes, needs or incidents relating to our loved one in care. At Heartland Health Care Center of Champaign, patients are monitored daily, including food intake. By the way, the meals are fantastic and the in-house culinary staff is absolutely the best. Meals are tweaked for day-by-day patient dietary needs, and the patient has the opportunity to select from among numerous menu items. Patients are scrupulously weighted to document weight gain or loss. A designated staff member will do roving blood pressure, heart rate and temperature readings multiple times during the patients waking hours. Medications are stored, maintained, allocated and distributed in a careful methodical way by a team of designated caregivers. The doctors, nurses and staff are cheerful, communicative, always positive about their relationships with patients. They also are careful to advise of patient progress, pointing out conditions which may necessitate important decisions later on, and delineating the options. The design of the facility is such that it is such that staff can monitor patients easily whether they are in their rooms, hallways, or commons areas. Comfortable chairs are provided in the commons as well as large screen TVs with cable (TVs are in rooms also). Telephones are provided in patient rooms - if necessary, communications may be channeled thru the nurses station. Family communications from afar can be conducted by email if so desired. The rehabilitation program and staff are unparalleled. They work diligently, exhaustively to restore physical and mental capabilities, not to mention enrichment in arts, hobby-related activities, etc. Daily progress is quantified, entered into a state of the art computerized data base which will be shared with family if requested. Doctors have office hours on site and are readily available to consult with family and patient whenever desired. Accolades to all, but especially the nurses and staff who deal one-on-one, day-to-day with patients in such a positive, friendly, caring way. They take all chores in stride, never hesitating, never wavering from the goal of the patient going home. Their patience and dedication are exemplary!

Our family did not have a good experience with Heartland. Our mother was moved to Heartland from Carle Hospital for end of life care. No one looked after her or checked on her unless we asked and even then it was hard to get a response. Heartland did not appear to have any record of her and there was little communication between shifts. We tried to give as much care as we could as a family because she was very uncomfortable and seemed to be in pain. She was supposed to be repositioned every 2 hours to prevent bedsores and no one came after the first time. Mom was in the facility for 36 hours before passing away and she was in misery the whole time. We had to suggest anti anxiety medication which they final gave at the very end. It seemed to us that should have been prescribed by a medical person sooner. The room was dreary and the bed was substandard. I would never send someone to this facility for end of life care.

Overcrowded rooms that are filthy. Staff always shorthanded. High turnover of staff. I have seen bloody bandages lying on floors and on window seals. Rooms so small that you can barely move if 2 beds in a room.
