Avoid this place at all costs. My wife and I went to get our I-693 from here. What should have been an hour long formality ended up being a two week ordeal. We were told that the form filling charges were $180 per person which is a bit steep but we went ahead with it anyway since we were able to get an appointment here earlier than anywhere else. The first appointment went fine, but towards the end the doctor says well have to come for another visit, and itll be an additional $140 per visit per person. I confirmed if that would be the last visit, and was assured that it would be. After the second visit, I was once again informed that Ill need to come for additional visits. This repeated up until the 4th and final visit when I was finally handed my I-693s for a grand total of $1,200. Oh, and payment is cash only. No receipt is provided unless you forcefully ask for one, and then youre provided a shady looking receipt that has no mention of the clinic or doctor, just a piece of paper with the date, your name, and amount paid written by HAND on it. I made the mistake of not pushing for a receipt for two visits - the receptionist told me shed give an aggregated receipt on my next visit. On my last visit, when I asked for the previous receipts she had the gall to claim I had not paid for two of the visits - and took the I-693 envelopes back from us. After a long argument she agreed to check the security camera footage. After an arduous hour or so of going through the tapes, we were finally able to find footage of me handing over the cash, and her counting it in plain view of the camera. She then claimed I hadnt paid for the other visit, and I again challenged her to check the footage at which point she finally conceded and wrote out an aggregated receipt (with the wrong date, no less). We got to the clinic that day at 2:15pm and left at 6:00pm. Not a single word of apology was given for wasting our time, and putting us through that ordeal. Im frankly amazed a clinic like this exists in a place like Seattle, it would be more fitting to find it in a dark alley of a lawless crime infested third world country. Avoid this place at all costs.

My husband and I went here to get our I-693 forms filled out, mostly because it was relatively easy to get in touch with them, and get an appointment (compared to other civil surgeons in the area). They seem to be reasonably efficient at filling the forms, and are quite accommodating. We received the (accurate) forms in about a week, and had to visit only twice (including a repeat visit to get our TB skin test results read). The receptionist+nurse+billing department, Madeleine, even offered to come in on a weekend to get it done faster. However, be sure to confirm costs upfront, and do not get vaccinations here if you can avoid it. I got my Tetanus vaccine here, and received verbal confirmation that it would cost about $55 (the same as in a drugstore). A month later, I received notification of an insurance claim for $55+$75(admin fees)+$79(office visit). Apparently this is standard practice for a vaccination at a doctors office, but still came as an unpleasant surprise. Additionally, there was a "accidental" $145 claim for an office visit. They have agreed to retract the claim, but their actions do not inspire trust

The worst clinic I have ever been. I went for the i-693 and they made us come 3 times, and wanted to charge every time even if we had to go because they forgot to tell us to fast for the blood test or because they did not have the results yet. They do not accept cards neither checks, but they do not tell in advance when you make the appointment. They made us a complete whole blood test even if it was unncessary for the i-693, but didnt tell us it was unncessary (we discovered it later because of other friends that took the exam in other clinics), so that they could charge more. We even had to wait in one ocassion for more than 2 hours. I would for sure never go there, overall if I had a serious medical condition.

Came here for I-693 exam – arrived for 5pm appointment to check TB skin and syphilis test results, left at 7:30pm. Charged 3x the cost for vaccines compared to Walgreens, the office doesnt keep payment records (patient in front of me disputed a payment and the staff had to review video camera footage to verify that they paid), will nickel and dime you for everything you need. The only person staffing reception is the one nurse, who is stretched thin between managing the reception and also performing the required tests. Very sketchy business practices – demanded that all payments be made in cash. The saving grace is that they managed to actually issue the form.

Excellent service! The team at the clinic proved to be highly efficient, systematic and most importantly, very reliable - leaving no stone unturned, they did an impeccable job of a process that would normally be quite challenging. I wouldnt think twice about turning to them for help anytime.

Quite a bad experience. Please stay away! Went for i693 - they charged almost 1000 dollars apart from insurance and quite unprofessional in keeping up any appointment.

Never ever ever go to this clinic please. They will not release your medical history if you change new doctor.
