I schedule an appt. for a well physical, not a school physical and since my arrival, there were many problems with the appt. First of all, the Nurse Practitioner Janet Slawinski did not have the courtesy of coming into the exam room and explain or even inquiry about my sons appt. There was a medical assistant who kept coming into the room to find out why my son needed a physical exam. The NP continued to tell the medical assistant about the difficulty it was going to be to do a physical with no school papers. I never requested a school physical, all we needed was a well physical, which is covered in full by the medical insurance. This health care provider kept talking to the rest of the staff about this physical she needed to do with no paper work. All this time the door was open, there was no privacy. Every single patient on the office knew that my son was there for a physical. The were many Hippa violations with this past visit. I do realize that we were missing the immunization records, but my main concern was to ensure that my son received the care he deserved. after all, this was a covered service. The insurance company does not have a clauses that specify the well physicals must be "school physicals only". I further explained to the medical assistant that if we were going to need a school physical in the future, my son can do it at the school at no charge. My son does not participate in any sports, therefore, the physical can be done at the school setting. It is frustrating that about 3 weeks ago, I took my daughter Jennissa Casillas for a "well physical", and we did not have any problems. The PA Stephanie Tomich took excellent care of us and never even asked for school physical. The PA did advise that we can bring the immunization records at a later time. The office failed to provide good care to the patient, there were several Hippa violations within the staff. The patients name and age was divulged in the office while there were several other patients waiting. I am not sure if your office follows Hippa protocol and how protected is patient information. The entire office staff needs to be trained on the difference between a "school physical" and a "well physical". Your staff cannot pick and decide who will have a physical and who will be sent home. This is not a professional approach to take with patients. The provider of care: Janet Slawinski, NP, it was so sad and worrisome to see a provider of care act in such a poor manner. As an advanced practice nurse, her job was to ensure that my son was ok . She never entered the room and introduced herself, or even explained to the patient and parent why she cannot do the physical. My impression was that she just does not want to perform her job. My entire family are patients of the practice. We never had any problems, service was excellent. Until now, when something like the above incident changed our entire perspective of the practice. Dr. Goldman, I hope that you have the opportunity to read this and discuss with your staff about ways to improve your practice. There is a need to teach your colleagues about Hippa, respect for others and most important, kindness to everyone.

Learn more about the vaccines, immunization schedules and what to expect at well child exams. Your childs actual vaccine schedule may vary slightly from this schedule, depending on vaccine availability and timing of previous vaccinations. If you have any questions, please ask your health care provider. *Screenings Beginning at age 3 annual check-ups include: Physical examination Weight and height Blood pressure Body mass index (BMI) Vision Hearing Health questionnaire Developmental/behavioral/mental health assessment Vaccines **Tdap: Teens 13 through 18 years old who missed the Tdap dose or received Td only at 11 or 12 years old should receive 1 dose of Tdap. †MCV: Teens who received the 1st dose of MCV4 at 11 or 12 years old should receive a booster dose at 16 years old. Teens who received the 1st dose at 13 through 15 years old should receive a booster dose at 16 through 18 years old. If the 1st dose was given at 16 years or older, a booster dose is not needed. ‡HPV: This vaccine requires 3 doses. The 2nd dose should be given at least 2 months after the 1st dose. The 3rd dose should be given at least 6 months after the 1st dose. this is what the American Pediatric recommend. seems like immunizations are important for both your child and the children they interact with- good idea to bring your shot record with you especially if the doctor has never seen your child before.....otherwise- how can they say- yes your child is good to go- thank you South Bay for making sure our kids are safe!

Went there recently due to a terrible stomach virus . They told me they would send my RX asap over to CVS. They actually forgot to do it leading me not to get my medicine that night. What is the point of going to a doctor if you cant get the medicine they prescribe? It was close to closing time and the office staff seemed like they were in a rush to get everyone out of there which is totally unacceptable. Also they intake form on the touch screen tablet takes way too long to do, especially if you are sick.

6 month pregnant wife. Sick with fever. First time here. They "forgot" to have her fill out the computerized sign in. After waiting approximately 1 hour and having to ask, we were informed they dont take our insuramce and that the wait going forward would be approximately one more hour. There were 7 people ahead of us upon arrival. 1 person with NO APPOINTMENT was seen before my pregnant wife. Staff seemed inconvenienced when asked a second time about the wait. Lousy bedside manner. Never returning.

Have been going for years. Friendly staff and great doctors. They get pretty busy for this reason but worth the wait.
