Front desk is beyond rude, did not know why i was there after showing her the email from my employer... had to call my employer to have them resend, then all of a sudden she had it 2 seconds after that.... She Was talking about patients with the other front desk rep that was with her, then a gentleman walked in behind me said" i need to bring you this copy " and handed her paperwork she then blurted out "oh whats this a fail?" completely embarrassing him with his response was " NO its not, i was told to bring you the copy" and she rolled her eyes and walked away. Then after sitting for 30 min i asked her if i could have a time estimate due to my kids having to be picked up from day care by a specific time, so i needed to know if i had to send someone to pick them up.. she then said " i will be called when i will be called, if i leave its an automatic fail" so i said ok and just made the arraignments ( good thing i did, i sat for over 2 hrs for a 20 min physical and drug screen) Then after being called back the lady who did my drug test had an attitude, but thats OK because she said "sorry if we all come of rude today because the people who have been here have pissed us off and have been ridiculous". One that is not my business about your other patients Two, DO NOT GET INTO A MEDICAL FIELD AND DEAL WITH PEOPLE ALLLL DAY IF YOU CAN NOT HOLD YOU COMPOSURE AND BE PROFESSIONAL!!!! GO FIND ANOTHER CAREER!!!!

I would have a hard time recommending this office to a dog owner much less a grown adult. The front desk staff shouts patients names, and also shouts why they are being seen. I also want to mention the statement below is a factual statement made by me the patient. I cannot attest to others experienced, only mine. I was forced to come here for a work comp. Evaluation. The doctor spent a total of 3 minutes and 45 seconds with me in 2 visits (I actually clocked him the second visit at 38 seconds.) He did no physical tests what so ever. He simply said I would be fine. I tried to state that i was having problems, and i quote "yes, that is expected". Then he walked out of the room. In my follow up appointment he never asked how I was, never told me what to expect going forward, and didnt reassure me. Frankly it was the worst experience I have personally had at a Doctors office. I previously was a Paramedic for 6 years, and have seen my fair share of poor facilities. This ranks in the top 10. The one redeeming factor is the Dr. is pleasant, and that is why they got 1 star. I should also mention total time waiting for my 3 minutes and 45 seconds approximately 3 hours. [I can only attest to the time seen because I actually timed the visit.]

There are so many things wrong with this 3 ring circus Im not sure where to start. Could it be the bearded male nurse that wreaked of cigarettes from 5 feet away? Or the receptionist doing her obnoxiously long fake nails during working hours? Or could it be the nurses and Drs (If you can legitimately call them that) that constantly state they are there to help the patient and not affiliated with Disability, workers comp, etc at all? Just another big red flag of how incompetent and wasteful big government is. My physical consisted of a bunch of questions, in which they knew nothing about my condition, 5 jumping jacks and walking 5 paces to the door and 5 back to the seat, sitting down and getting back up out of the chair. Thats it. They determined I was able to work after Ive been declared disabled for quite a number of years with various bouts of cancer and other severe life threatening issues that plague me to this very day. This place is a joke and I wouldnt trust them to take my temperature accurately.

Well, just came from 1.15 hour drive to get to Jet for workers comp therapy. Still dont have full active range of motion, hand numb, nerve pain. Poor functionsl use. Same exact issues plus more now. Dr Choski, berated me repeatedly for missing an appt. No one left me a message. He told me he closed the case because I hadnt been back for 2 months and missed an appt. I think seeing a doctor in 2 months while going to tx, IS continuity of care. Ive been going to therapy and doing it at home. I had a couple of personal issues that prevented me from completing therapy sooner, like being stuck out of town with car broken down. He didnt care. Didnt want to deal with trying to help me. He hadnt read therapy report and told me he wanted $25 for a disc copy. All I wanted was a 1-2 pg. zerox copy. SHAME ON DR. CHOSKI...(shaking my head).

Took my partner here for a workmans comp back X-ray. People are rude and unprofessional. Would not allow me to accompany my partner into the medical bay citing their "CEOs policy". Would not recommend people come here unless you are forced to. Edit : despite his assertion of a "nice conversation" (basically a bs scripted response to someone being unhappy) my opinion still stands. The overall atmosphere of the facility is negative and unprofessional. This was also not my first time in this facility and not the first negative experience. I was a patient approximately 2 years back and the staff did not treat me kindly or with respect.

I was forced to come there for a work comp claim! Dr. Choski is very unprofessional ! I hate this facility! I would rather wait 10 hours at US health works then to sit in his facility! The patient does not matter to him . If you are injured, he ignores the pain and the complaint you provide him ! I wish somebody would take his license so he would not be able to practice anymore! Worst Workers Comp doctor ever! If I was on my dying bed ,I wouldnt allow him to touch me !

Today I had a visit for disability,I was nervous for being first time,but everyone from the time I walked in to the time I left was very friendly,The doctor is very down to earth and talked to me then he said after the N.P. sees me if I want to talk to him after he will come back in.I know I am only there for a disability check up but they didnt treat me any different due to that.Thank you!!!

This doctors office is literally the worst doctors office I have ever been too. The wait times are ridiculous. I had an appointment at 11am, & have been waiting for an hour & half. I will never come back to this place ever.. The docotors here suck as well. All they want is to drug you up & get you outta there so you can come back & get drugged up again. Awful. Awful. Awful.

Bad experience with this organization. They lost all of my information on two occasions. They are unstructured and very unorganized. Staff does not seem to have a clue to what is going on. I was scheduled an appountment and when I arrived, no one knew what was going on. They wanted me to again give them same information that I had previously given twice.
