Everyone was very nice and accommodating, but the dental care I received was rather poor. Every filling Ive had here has fallen out, including one Ive had filled twice. My brother also had a filling done so poorly that he couldnt floss the neighboring tooth, causing it to fully decay and needed to be removed. This dentist honestly discouraged me from seeing any dentist for a while out of fear that they all would give poor treatment like I received here.

Dr. Formans office is very nice and clean.Staff is very friendly, professional, and knowledgeable. They explain the procedure step by step and show a video to make it easier to understand. They do not make you wait. If you have any questions about the insurance, the front desk is more than happy to help you. Dr. Forman and his staff are always nice and helpful in advising what is best for you. I would recommend this office.
