DONT EVER GO TO THIS DOCTOR. I came to his office for the Medical Form for the Immigration Application, but he filled out the wrong information in my form and didnt give me a required vaccine. My form was rejected by the uscis officer and I came back to him to fix it but he NEVER tried to help me. He kept telling me to come back to his office but when I came he never showed up. My application was delayed and I had to take off work and drove around the city to find another doctor to get another form within 24 hours after my interview.

I came here because I needed to get a quick physical and drug test and it got the job done. However my appointment was at 4pm and I arrived at 3:50pm and didnt get called back until 4:19pm. Also, they didnt let me know what was going on while there were doing the drug test so I was just sitting for 30 minutes waiting.
