Dr. Sack was probably one of the best doctors I have seen and he deserves his retirement; he will likely be sad to see his legacy come to this. 1. I put in for my refill early, giving you 4 days to take care of it before the holiday weekend. a. You made an active choice to do nothing with it because you wanted to collect an office visit fee. b. I often travel out of the country and need to know I can rely on simple tasks like prescription refills to happen consistently. c. This is unprofessional. 2. I had to find out on my own that your office chose to ignore the refill request. a. If the pharmacy, insurance company and myself are waiting for you to take action and you have decided to do nothing with it, you at the very least need to have an obligation to notify me of your malfunction. b. This is unacceptable. 3. After learning of the problem, I called during the extended hours on Saturday to get it resolved. Understand that if you neglect your duties during regular hours then I do not care if it’s party time on the holiday, you have unfinished work. a. Your employee told me they were going to do nothing with it until after the holiday, refused to connect me to the doctor and then hung up on me. b. This is unprofessional, unacceptable and requires reprimand. 4. After abruptly stopping a medication I have been on for 2 years and now 7 days behind, I call in. a. I notify you that my prescription has not been filled, and that a prior authorization also needs to be sent to the insurance company. b. Apathetically a day later you send a prescription to the pharmacy that can’t be filled because the prior authorization was not done. c. The only competent person in this process has been your receptionist who has taken detailed notes, and explicitly gave you the message to complete a prior authorization. d. Now 10 days late, we start that process which at a minimum will take the insurance company 48 hours to complete and likely will be delayed by the pending weekend...again. 5. I have had to call daily to hold your hand and walk you through this like a baby taking their first steps. a. If you are unable to read, unable to communicate and unable to follow instructions, for what reason should I trust that you can make sound judgement on the complexities of the human body; I’m at a loss there.

Called at about 8am, tried to get one prescription approved I had already called my walgreens to send the request so i thought let me call my physician and give them a heads up. The operator let me know the dr would be approving it as soon as possible she confirmed it would be done by the same day. Called back to my pharmacy about 12pm nothing yet, come 4:45pm i checked with my pharmacy again and nothing. So i called my physician the second operator i spoke to was very rude, kept asking me the same questions over and over again as if I was lying. I told her if it was going to be this hard to get it done i should of just gone in and see the doctor letting the first operator know i dont mind going in knowing Dr sack wouldnt be there and a new dr would take his place i figured he/she would want to see me in order to approve it but she said she was going to try to have the new dr approve instead. I wasnt able to get any names of the operators which is frustrating. the second one kept just trying to push me to a manager that i didnt want to talk to she kept telling me i was upset which at the end i was just pissed it was closing time and she didnt want to deal with me i just asked her whats the deal with all the pointless questions she kept asking me that i had already answered. Point is i called early enough to think my stuff would be appoved and what the second operator told me was that apparently no one even looked at my stuff or tried to approve it.! (By the way thats not something you want to tell a patient from experience) Horrible horrible i cant believe its that hard to push a paper infront of a dr. At the end i let her know to just forget it i was going to end up having to change physicians she simply said "okay thats fine good bye"

I will say they did remove all the charges, however this is the bill the insurance company would have been sent... AND WE WONDER WHY INSURANCE IS SO HIGH... DO NOT USE DR CYNTHIA LEW... I went in to get a prescription refill because they would not refill without me coming in. Our insurance had not kicked in so I told them I was self pay, Dr. Sack would charge me about 75.00 for this kind of visit! I was charged $251.00, NO BLOOD WORK, SHE LISTEN TO MY HEART, AND HAD ME BREATH. That is all that she did besides telling me I needed some test, test that I have never heard of... I was charged $20 for TELLING me I needed to take it. $30 for TELLING me I needed it, and $6 for TELLING me another one.... $160 for office visit (I was told on the phone it would be $82.50 for being a new patient) I did get a flu shot $35... This is unbelievable myself and my husband called, I was told she could charge me for anything she wanted! I miss you Dr Sack, I have found a new Doctor he is great, Dr. Doan, cost $170 (new patient and full blood work up) And he even told me he wanted me to take some test NO CHARGE!!

Update: after speaking with someone from the main office I went back and gave them a second chance. I saw Dr. Bernstein , a wonderful Dr. and a compassionate man. This was 10 days ago. They got on my refills right away yesterday as I ordered them through the patient portal. So I will definitely continue going back. After 20 plus years I like everyone else was sad to see Dr. Sack retire but he gave many more years than most before he hung it up. I was prepared to give the new practice a shot but was informed the new Dr. would not treat me for my main health issue. I was told that I would need to see a specialist , which I have already seen over the years. It took many years of trial and error to get this under control and I know all Dr.s have their way of practicing medicine but that said if its working and working well dont change it. Theres an old saying if it aint broke dont fix it I think that applies very well to this situation. And I believe if you look at the number of patients that have left the practice since Dr. Sacks departure youll see Im not alone in my way of thinking.

Never in my life have I had such a hard time getting my prescriptions filled. Ever since Village Family Practice took over Dr. Sacks clinic, it has been a nightmare trying to get my prescriptions every month. When you call to try to get something done about it, you dont even speak to someone in the clinic. You are speaking to some answering service that is completely useless and never helps in correcting the problem. At the beginning of this week, I called to get my prescription refilled and have yet to have that happen. I was just called by the clinic and Im thinking that finally I am going to get it filled, only to be told that my doctor works at a different clinic on Fridays so it more than likely will not be filled until Monday. Now I will have gone without the medication that I really need for an entire week! I think that it is time to start looking for a new doctor, I CANNOT keep doing this every month.
