Needed an FMLA form to filled out to cover my missed time at my job, the first day, 2 weeks ago I talked Araceli (the branch manager) who said I could email the forms and they would be sent back, 2 days later I spoke to the Dr. who said to resend the forms, then all of a sudden I was told it would be a $45 charge just for him to sign 3 pieces of paper, I asked Araceli why was I being charged since 2 days ago she never mentioned a cost, her response as the branch manager was, I didnt know, I told her I had to save for the money because I had not been working, a few days later I called, she told me the charge was $35 which is more than my monthly Obamacare premium mind you, just for 3 pieces of paper, then I was told to give him till over the weekend, which was understandable, I was told, if I didnt get it by Monday night I would get it by 11am on Tuesday, I called Monday, was told Katie was sick and not in the office and would be in tomorrow, I asked why was I waiting for Katie to fill out a form Dr. Arizmendi said he was gonna fill out himself. She had no answer, she repeatedly says she is trying to help me, but help me how, by giving me the runaround? I called at 11:40am, was told the Dr is busy, I get it, but was told I would get a call back around lunchtime...surprise, surprise no call, I called back at 3:30pm on hold for 15 minutes just to be told, they would be completed at 5pm, even though I needed by 430pm before HR closed, which closed at 4pm. The lady kept requesting $35 payment, but the Dr. failed to put the dates I needed which was 2/25 because I went to the hospital that night but they didnt enter my paperwork until 15 minutes after midnight after waiting hours to be seen, until 3/14 to cover the 4 days after I met Dr. Arizmendi at my follow up due to a medical complication reaction from the IBS medication he prescribed that was doubled by the pharmacy bc they were on back order for the dosage originally prescribed. At 4:07pm today I get a call back from the office saying something that took days was completed in 15 minutes, and they wanted the $35 to cover 2/26-3/9, which doesnt cover the time I needed, which is total bullshit, had I not taken the double dosage pills I could have been fine to work that weekend, but nobody suffered rectal bleeding and excrutiating stomach pains like I did. And his office is closed on the weekends, since meeting him, I have been back to the ER 2 times. Very disorganized office, very rude staff, and I will be contacting my insurance and the medical board as Obamacare patients shouldnt have to pay more than their monthly premium for 3 forms. The forms were to cover my time so that I dont lose my job, but clearly this office does not care for its patients, I would go someplace else less "busy" and more organized. Will be filing several formal complaints.

A couple years ago my sister recommended Dr. Arizmendi to me because I was unhappy with my previous doctor and his office staff. I live southeast of downtown SA but in my opinion Dr. Arizmendi and his staff are well worth the long drive for me. The staff is so caring and cooperative. Dr. Arizmendi reminds me of "old school" doctors in that he listens, knows what he is talking about, and earns a patients confidence. I would gladly recommend him and his office staff and nurses to anyone.

Dr. Arizmendi is very caring and understanding. In this busy world that we all live in he makes sure he takes the time to answers your questions and concerns. I only wish there were more Doctors in this world with Dr. Arizmendis values!!
