Room is dirty - first day observed dirt piled up in corner and an old washcloth in the room behind his bed. Lots of cob webs under ac/heater unit which is very visible. Bathroom shared between 2 rooms (up to 6 patients), and someone always seems to be in there. Smell throughout facility is terrible. Father was very uncomfortable and sat up, aide come in and said "what are you doing! You got up, you can get back down yourself!". On another occasion, an aide was changing clothing...raised the foot of the bed and lowered the head making my father inverted and left the room for almost 30 minutes...not just 2-5 minutes! I have sat up there with my father for 3 hours and never seen an aide or anybody. Mother has witnessed the same. Right now it has been 2 hours and 35 minutes and no visit from staff. Floor molding coming off a couple walls. Very outdated dressers and bed trays, looks very worn/dirty. We went searching other facilities and found others, 10 fold better! We were told we needed a social worker to send the other facility a referral. Since its Saturday we have to wait until Monday. Nobody on call! We were told we can ask to talk to the manager on duty, they are not available either. They said they would send a nurse down...guess what, they didnt! Expectations are very low for any requests at this point. Just want to get our father the care he deserves and hope others will read this to be thorough with their selection. I was very honest and to the point when I discussed moving my father with the social worker. My brother and I made many of the calls to expedite. Communications in the healthcare industry are terrible! Its not all negative. The therapists and nurses during the day are very friendly and good at what they do. When they leave, all patient care subsides! We are currently at another Heartland in Canton, what a difference! A little slow to respond, but seems like that at all these type locations. Not enough workers for number of patients?

Others have described the terrible odor. My loved one has been a long-term patient at this facility for several months. Early on I noticed a few issues, but ultimately decided to give them time before I passed judgement. TIMES UP!! First, my loved one is on a restricted diet, however, they consistently serve him restricted foods. Second, I had ONE DAYS notice that my loved ones Medicare coverage was to expire and had to scramble to assure that long term care would be provided. Third, my loved one is on dialysis three times per week. Several times Ive received calls that the transportation company contracted by Heartland failed to return to dialysis center once the treatment was completed. Finally, during a recent visit to my loved one I discovered that their shower schedule (only 2x per week) conflicts with the dialysis days so there has been no bathing in three weeks. I am OUTRAGED and will be transferring my loved one as soon as possible.

This is the worse nursing home Ive ever stepped foot in. Ive been to quite a few being a caregiver for 5 years and with my dad. The care is awful. I watched one woman sit for 6 hours with not one check. I wish I knew her family so I could tell them. Of course I told the person in charge but it did nothing. I wont even start on the urine smell when you walk in the door. Ive never seen staff so rude to the elderly a day in my life. It hurts my heart.. .Do yourself a favor and take care of your people at home. My next step is to call whomever owns this dump and tell them about their business.

I was a patient at Heartland-twice-and my experience was amazing! The Nursing Staff is very friendly, and they are like second family. The therapy department is second to none! High energy, positive, and motivating! They assist in your physical, mental and emotional well-being. I would recommend this place to anyone who are serious about their rehabilitational goals. I was a little sad when it was time to leave, but I know I was equipped to progress with my health needs.

The nurses here are not educated on what the signs and symptoms are related to Sepsis. My mom had altered mental status, she was alert and oriented during her admission the previous day, the nurse did not check her vitals, call her family, or even call the doctor. She told me "I noticed she was different from when I admitted her yestersay" yet she did nothing to act on it. My mom died a few days later at the hospital. This place disgusts me.

Great stay here! Physical Therapy program was great, and all employees in physical therapy were awesome! The staff in this facility was fantastic! I was greeted every day with a smile Kudos to nurse aides Daniel and Mary. They really care, and went the extra mile for me every day. Thank-You!

This place isnt.the best my girlfriends grndma is.there.and her room isnt kept. Clean there was old tubes they had used left lieing around nurses left.theres wrappers from needles in her room just really poor effort in the nurses and any help. In maintaining a nice living arrangement

Filthy! Not ADA compliant in patients bathrooms. There are flies everywhere! Awful feces odor. Beds are so old that they sag in the middle- porous wood headboards.
