I have been a patient of Dr. Tormohlen since 1998. He was my childrens doctor as well. I have always received optimal, quality care from him as well as from Dr. Quintana and Melissa. He also cared for my husband for 8 years before he passed. Dr. Tormohlen is not only a wonderful doctor, but is a kind individual who cares for ALL of his patients. I am saddened by the negative reviews I have read as it does not describe my doctor, nor the other medical staff that I know well. If there is an issue with office staff personnel, then it will be addressed. This team of doctors puts their patients first. I have witnessed it over the years, as well as have spoken with many who agree that we are a blessed community to have such a dedicated team of doctors to serve us. If it were not for Dr. Rick Tormohlen, I may not have pulled through having the health conditions I have lacking health insurance at times. I have reversed my health by reducing weight, reversed diabetes, lowered my BP and quit smoking. My doctor never gave up on me, and encouraged me through education and support through it all. My husband also was treated for complications due to his MS, and Dr. Tormohlen was always quick to respond to his medical needs, and even saw him in the hospital. No one has to wait 45 min in the lobby..if you have checked in and waiting, ask them for assistance..They too are wonderful. Over the years office staff has changed like most every where else because only the good ones will find a home there. I am grateful to my doctor for the many years he has taken care of me, and pray he practices long enough to take care of me in my old age. The reputation of Forum Family Medicine speaks for itself as they are growing, and have added a new PA because of their high expectations, quality of care and excellent customer service. If you have a billing issue, please dont blame my doctor. They need to get paid just like everyone else to continue providing good care to us all.

Patient care and staff interaction was abysmal. My husband went in for a routine exam with Dr. Tormohlen and got back some results that were not within the normal range. The doctor never called to report his results to him, but instead we discovered them over a week later when we logged on to look at his results. We had to google the results and then call the doctor to try and schedule a follow up appointment. Days would go by between inquiries or we just would never get a call back. On the second appointment, the doctor requested a second blood test. He did not perform any of the other checks that my husband originally wanted and that he said he would do on this visit. After the appointment, his staff called with the results this time, but could not answer any questions. We again did not get a reply from the doctor. By this point, my husband was upset and left a voicemail stating such, though admittedly, heatedly. We just had a baby and are trying to find out what is wrong with him and could never get any answers or direct information from a doctor or medical professional. Rather than the doctor calling, we got a letter of discontinuation of care. Which is fine by us... we had already made an appointment to go elsewhere. Very disappointed, especially considering some of the good reviews we had read. Additionally the doctor never made eye contact with my husband when discussing his care and was very impersonal. After the first visit my husband said he didnt think he cared for him, but said maybe he was having an off day. The interaction did not improve on the second visit. The other doctor was booked out by about a month, whereas Dr. Tormohlen was available within the week. Now we know why.

A very uncaring office since they "updated" thier billing system. I was a patient of of Dr. Phillip Quintana about 15 years! When I was extremely sick with the flu I called to schedule an appointment to be told in a very uncaring way that I had a unpaid balance from "2007" and would need to pay that before I could see a Dr, or in there case an assistant. I had been to see them before and was never made aware of such a bill. I have good insurance so I hung up, called my insurance, they took care of it. Found another Dr to go to, ****month later I get a bill for a "missed appt"!!! *****They had scheduled me that day even though I told them I could not pay and would call my insurance.!!! Think twice before you mess with that practice!!! Billing System is not correct

When I tried to call to reschedule an appointment that was first thing in the morning (even before they turned their phones over) due to sick babies and sick myself(which was not the reason for the appt.), the response I got from the office staff was, "we ARE a doctors office--you can bring your sick kids in--we require 24 hours notice so you will be charged $45.00." Really? In a business that deals in healing and compassion, they certainly are very uncompassionate! I understand that they lose money when they cant refill an appointment slot, but they need to allow for peoples lives and extenuating circumstances.

The staff is very rude and I always feel like I am bothering them. The front desk staff wont even greet you when you walk in. It makes me very sad when my kids need to see a Doctor that they are very unconcerned. Went to urgent care and paid more just to not deal with the ladies at the front desk. It is a very uncaring atmosphere, they must hate their jobs or people. Either way I will not be back to see these Doctors for that reason. Sad!

Customer service is poor. Youre not acknowledged as you wait at the desk as the person finishes their paperwork. Waited 45 min in lobby after signing in then she didnt have anything in the system fo my appt. Finally seen. Get a msg at home asking if I wanted to reschedule since I missed my appt. Hmmm- thought you said you didnt have me in system. Also parking could be a problem. Doctor Quintana is good and Melissa (PA) shes wonderful.

Most unprofessional staff. Avoid this place at all costs
